Combination of Healthy Diet & Healthy Body Weight Lowers Cataract Risk

The Blue Mountains Eye Study showed that carefully following dietary guidelines decreased risk of visual impairment. However, whether or not this association extends to age-related cataract was not clear. So, researchers have now reported on a study to determine if strict adherence to dietary guidelines, using total diet score, impacted the incidence of age-related cataract. Using complex statistical analysis of the Blue Mountains Eye Study, the found that adhering to dietary guidelines and total diet score by itself was not associated with incidence of any cataract, BUT as total diet score increases with a Body Mass Index (BMI)<25, there is an association with lower risk of nuclear cataract which is a known aging marker.

Glaucoma Risk & Cholesterol Medication

Glaucoma, a leading cause of blindness, is a condition where pressure builds up in the eye and affects the optic nerve. Recent research suggests that statins-cholesterol-lowering drugs prescribed to treat and prevent cardiovascular disease-also lower intraocular pressure and promote blood flow to the optic nerve, which may help lower glaucoma risk.

Researchers from Brigham and Women's Hospital in Boston have reported on a study in JAMA Ophthalmology demonstrating that taking statins for five or more years is associated with lower risk of primary open-angle glaucoma. Their study suggests that there are possible protective associations beyond cardiovascular conditions offered by long-term statin use. Statins may strengthen neuroprotective mechanisms that prevent degeneration of cells in the optic nerve. The results showed that use of statins for five years or longer, versus never using statins, are associated with a 21 percent lower chance of primary open-angle glaucoma and that elevated cholesterol levels may heighten glaucoma risk. They also cautioned that the findings do not mean that individuals with family histories of glaucoma should use statins or other cholesterol medications for glaucoma prevention. Randomized clinical trials will be needed to determine if a causal link exists between statin use and glaucoma prevention before physicians can recommend statins for lowering risk of primary open-angle glaucoma. Particularly in elderly populations, statins have potential side effects, including risk of muscle damage and liver or kidney dysfunction.

Visual Perception in Kids with Epilepsy

Brain surgery for epilepsy in kids is sometimes necessary to stop seizures and allow children to function. However, brain surgery carries significant risks, including impairment in visual perception. Normal visual function requires not just information sent from the eye, but also image and neurological processing in the brain that allows us to understand and act on that information, or perception. Signals from the eye are first processed in the early visual cortex, a region at the back of the brain that is necessary for sight. They then travel through other parts of the cerebral cortex, enabling recognition of patterns, faces, objects, scenes, and written words. In adults, even if their sight is still present, injury or removal of even a small area of the brain’s vision processing centers can lead to dramatic, permanent loss of perception, making them unable to recognize faces, locations, or to read, for example. But in children, who are still developing, this part of the brain appears able to rewire itself, a process known as plasticity.

According to a study funded by the National Eye Institute (NEI), part of the National Institutes of Health (NIH), due to this neuroplasticity- a sort of “rewiring” process that can occur in children because they are still developing, children can keep full visual perception—the ability to process and understand visual information—after brain surgery for severe epilepsy.

Stephen B & His LASIK Experience

“I recently received a second opinion for LASIK surgery when my primary ophthalmologist suggested PRK. PRK has about a 3 month healing time, while LASIK is roughly 12 hours. Dr. Confino determined I was actually eligible for LASIK surgery, which I ended up doing on site since he has his own laser. The service I have received from his ENTIRE staff has been completely top notch, including his Director of Surgery, Jerette. I couldn't be happier. When I walked into Dr. Confino’s office, my vision without glasses was measuring at what they call, Count Fingers, meaning that I could not even see the big E on the eye chart and could only tell how many fingers were being held up close to my face. Glaucoma & Risk of Falling from Visual Field Damage

The visual field loss resulting from the progression of glaucoma can pose a risk of falling to patients. Falling and the fear of falling (FoF) is a significant risk to patients-especially seniors. Researchers were interested in finding out more about how and when patients with visual field damage from glaucoma were at risk of falling and thus reported on a study in the American Journal of Ophthalmology in which they investigated where glaucoma patients most often fall and how integrated visual field (IVF) damage affects falls rates per year (falls/year) and per step (falls/step) at and away from home.

Their research conclusions for this glaucoma population suggested that a) most falls occurred at home, b) the risk of any step resulting in a fall was higher at home and c) those with greater visual field damage were more likely to fall for each step taken both at and away from home. Based on this, it is important to consider making efforts such as home environmental modification to prevent falls while maintaining physical activity.

How Common is Dry Eye Disease?

Dry eye disease (DED) is a multi-factorial ocular surface disease characterized by patient complaints of dryness, grittiness, discomfort, irritation, and often visual disturbance. DED can create a significant burden for patients, including problems in social settings, occupational functioning, and reduced quality of life. Known risk factors for DED include female gender, increasing age, and certain systemic health conditions (i.e. autoimmune diseases), and a wide range of medications that are known to result in dry eyes. It is estimated that some 5% to 34% of individuals over 50 suffer from dry eyes depending on the definition of dry eye used to prepare the estimate.

Lazy Eye Risk in Premature Children

There is generally concern regarding vision problems in premature children and especially those with retinopathy of prematurity (ROP). Researchers investigated whether the incidence rate, cumulative incidence, and prevalence of any amblyopia risk factors were greater in retinopathy of prematurity (ROP) and non-ROP screened groups of children up to age 3. The incidence of amblyopia risk factors was not significantly different between non-ROP and ROP screened children however, the prevalence of refractive errors among premature children-especially astigmatism- was higher than that reported in childhood in the eye care literature.  Based on this, it may be appropriate to screen all children with a history of prematurity for refractive errors around 24 months of age. If a friend, relative or acquaintance has a child born prematurely, it might be worth suggesting that they schedule a screening eye exam by age 2 if they have not already done so at the direction of their pediatrician.
