Could An Eye Doctor Diagnose Alzheimer’s Before You Have Symptoms?

A recent study published by Duke University researchers in the journal Retina suggests that an observed loss of blood vessels in retina may reflect changes in brain health and could signal Alzheimer’s disease. In people with healthy brains, microscopic blood vessels form a dense web at the back of the eye inside the retina. In the eyes of people with Alzheimer’s disease, that web was less dense and even sparse in places. The differences in density were statistically significant after researchers controlled for factors including age, sex, and level of education. The study measured blood vessels that can’t be seen during a regular eye exam using noninvasive technology that takes high-resolution images of very small blood vessels within the retina in just a few minutes, called Optical Coherence Tomography-Angiography (OCT-A). They concluded that it’s possible that these changes in blood vessel density in the retina could mirror what’s going on in the tiny blood vessels in the brain, perhaps before we are able to detect any changes in cognition. 

With nearly 6 million Americans living with Alzheimer’s disease and no viable treatments or noninvasive tools for early diagnosis, its burden on families and the economy is heavy. Scientists at have studied other changes in the retina that could signal trouble upstream in the brain, such as thinning of some of the retinal nerve layers. The goal would be to use this technology to detect Alzheimer’s early, before symptoms of memory loss are evident, and be able to monitor these changes over time in participants of clinical trials studying new Alzheimer’s treatments.

Heavy Smoking and Vision Loss

Researchers at Rutgers University Behavioral Health Care reported findings of a study in Psychiatry Research demonstrating that smoking more than 20 cigarettes a day can damage your vision. The Centers for Disease Control and Prevention estimates that 34.3 million adults in the US currently smoke cigarettes and that more than 16 million live with a smoking-related disease, many of which affect the cardiovascular system. Cigarette smoke consists of numerous neurotoxic compounds that are harmful to health. These neurotoxic chemicals are believed to result in nerve damage to the optic nerve and retinal nerve fiber layer resulting in a loss of color vision and contrast sensitivity. This damage is in addition to the known harmful effects of smoking which doubles the risk for age-related macular degeneration (AMD) and hastens the yellowing of the crystalline lens of the eye where cataracts form.

Deep Learning & Macular Degeneration Risk Assessment

Age Related Macular Degeneration (AMD) is a leading cause of vision loss in seniors. Aging is the greatest risk factor! In its most advanced stages, it can deprive an individual of his or her ability to perform basic activities such as reading, recognizing faces, and driving. Approximately 11 million individuals are affected with AMD in the United States (U.S.) alone and the prevalence of AMD in the U.S. is anticipated to increase to 22 million by the year 2050.

Early detection and treatment is critical in helping patients avoid vision loss from AMD. Recently researchers reported interesting results in the journal JAMA Ophthalmology regarding how “deep learning” (DL) or artificial intelligence (AI) might be a useful addition to the physician in the examination and monitoring process for patients at risk for AMD and its progression. Their findings suggest that using DL to grade and monitor AMD is comparable to experienced humans for estimating 5-year risk of progression to advanced AMD. This could be very helpful in assisting physicians in having a detailed risk assessment in the long term care of AMD patient.

Fat Transfer Procedure Instead of Fillers?

Blame it on your 5 A.M. spin class or missing your morning coffee because of the long line, but the exhaustion written on our faces isn’t pretty. When concealer isn’t enough, you may wonder what cosmetic procedure would help restore that youthful glow to your eyes. 

New Jersey Eye Plastic Surgeon James Chelnis, M.D. at The Eye Care & Surgery Center in Westfield shares information on fat transferring as a technique for facial rejuvenation.  

“You might be surprised that fat transfer is still a popular technique with fillers being so popular today. Modern fillers are definite upgrades from the previous generation technology. Fat transfer is a time-tested technology that uses your fat and puts it where you wish it would be! Since it comes from your body, there is no chance that you would be reactive or allergic to it. If you travel a lot and have a want to be free of repeat appointments for facial fillers, fat transfer could be an excellent option. The procedure usually uses fat from your abdomen but there are other good sources too, such as from certain parts of your thighs or flanks. This is determined prior to the procedure with your physician. During the same visit, the fat can be added to the targeted area to complete the treatment. Fat harvesting is typically done through a tiny site a few millimeters wide and heals very quickly as a result. Technical elements to the procedure include how to prepare the harvested fat for injection. There are many options for harvesting including choosing a donor site, selecting the proper instrumentation for harvesting and injection, and fitting the procedure into the patient’s larger aesthetic goals. One of the most common and effective places to use fat transfer is in the area around the eyes, particularly when hollowing and shadowing under the eyes. There isn’t much in the way of preparing for fat transfers outside of normal steps before any procedure. This includes pausing blood-thinning medications and supplements before the procedure and keeping away from strenuous activity for a short time after the procedure. Typically, fat transfers can last much longer than fillers, sometimes for many years, and as a result, in the long term can be much less expensive than fillers. This is great for patients that want something resembling a “one and done” approach to aesthetics. Since it is a medical procedure, there is a recovery period, but not as would be seen from a more invasive surgical option. Patients should expect short term soreness or bruising from the donor site and some short term swelling in the recipient site.”

Laser Treatment Can Be First Choice for Glaucoma

Researchers reporting in The Lancet on the LiGHT Study concluded that a change in the clinical practice pattern of how we treat glaucoma should be implemented so that glaucoma laser treatment was a first choice treatment, rather than eye drops which is the typical approach to glaucoma treatment. The LiGHT Study compared eye drops to glaucoma Selective Laser Trabeculoplasty (SLT) treatment in terms of lowering intraocular pressure, health related quality of life, cost and cost effectiveness. The data was gathered and analyzed and concluded that Selective Laser Trabeculoplasty (SLT) should be offered as a first-line treatment for open angle glaucoma and ocular hypertension, supporting a change in clinical practice.

Microscopic Stents Help Lower Eye Pressure in Glaucoma

Glaucoma is the leading cause of irreversible blindness worldwide, and its prevalence will continue to increase as our population ages. For most patients, the initial treatment for glaucoma will begin with eye drops or laser treatment, or even both. Patients often have a difficult time adhering to a schedule of single or even multiple doses of eye drops and thus find an initial laser treatment beneficial. Sometimes even with eye drops and laser treatment for glaucoma it is not possible to lower the intraocular pressure enough to reduce the risk of vision loss. For these patients there has been the next step of complex incisional glaucoma surgery. Recent advances in Minimally Invasive Glaucoma Surgery (MIGS) using microscopic stents has been shown to provide excellent results for many glaucoma patients. Reports in the Journal of Cataract & Refractive Surgery of patients who had iStent-a tiny titanium stent-implanted through a microscopic incision, had excellent pressure lowering and even were able to reduce their need for eye drops! The data shared was from a long term 5 year clinical study and offers great promise for glaucoma patients.
