NJ Specialist on Glaucoma & Medical Marijuana

Medical marijuana is promoted as a treatment for many diseases, including glaucoma. NJ Glaucoma Specialist Roslyn Stahl, M.D. of The Eye Care & Surgery Center in Westfield, Warren & Iselin, New Jersey discussed medical marijuana and glaucoma. “Glaucoma is an eye disease in which the optic nerve becomes damaged over time, reducing peripheral vision. If not properly diagnosed and treated it can lead to blindness. One cause of the optic nerve damage in glaucoma is higher-than-normal pressure within the eye, called intraocular pressure or IOP,” explained Dr. Stahl. “Currently glaucoma treatment for the most part is to lower IOP levels with eye drops, laser treatment, oral medications and sometimes surgery depending on the type of glaucoma and how severe it is.”

The idea that marijuana can be helpful in treating glaucoma dates to the 1970s. Studies conducted then showed that smoking marijuana lowered the IOP of people with glaucoma. As a result of this research, additional studies were conducted examining whether marijuana or its active ingredient, a compound known as THC, could be used to keep IOP lowered. This research was supported by the National Eye Institute, a division of the National Institutes of Health. The research found that when marijuana is smoked or when a form of its active ingredient is taken as a pill or by injection, it does lower IOP. However, it only lowers IOP for a short period of time-about three or four hours. This short period of time is a major drawback for the use of marijuana as a glaucoma treatment. Because glaucoma needs to be treated 24 hours a day, you would need to smoke marijuana six to eight times a day around the clock to receive the benefit of a consistently lowered IOP. Because of marijuana's mood-altering effect, smoking so much of it daily would leave you too impaired to drive, operate equipment or function at the peak of your mental ability.

The effect that marijuana has on your ability to function is not the only side effect from the drug, however. Marijuana cigarettes contain hundreds of compounds that damage the lungs. Research has shown that regular and frequent use of marijuana, particularly in high doses, can cause problems with short-term memory and concentration.

As scientists learn more about glaucoma, they have also come to understand that IOP is not the only factor that damages the optic nerve. Recent studies characterize glaucoma as a neurologic disease similar to Parkinson's or Alzheimer's. And, there is increasing evidence that reduced flow of blood to the optic nerve may also cause damage in patients with glaucoma. Unfortunately, marijuana not only lowers IOP, but it lowers blood pressure throughout the body. As a result, it has the potential to lower the blood flow to the optic nerve, effectively canceling out the benefit of a lowered IOP. So, while marijuana can temporarily lower your IOP, it's not recommended for treating glaucoma. Prescription medications, laser and surgical treatments have been tested and proven as effective treatments for the glaucoma.

When to Exercise after Cataract Surgery

Today’s active cataract patients should know about the guidelines for resuming exercise after cataract surgery and lens implants. “In reality, the cataract surgery and lens implant operation allows for a quick activity and visual recovery,” stated Cataract Surgeon Joel Confino, M.D. of The Eye Care & Surgery Center in Westfield, Warren & Iselin, New Jersey. “This is pretty important to more and more patients-especially as they want to be able to resume their daily activities and lifestyle quickly. One area we always discuss with patients is how long they need to wait or restrict their exercise routine,” Dr. Confino further explained. “Certainly exercise guidelines may vary slightly depending on your cataract surgeon and on the specific details of your cataract surgery, but in general within 24 to 48 hours you should be able to do light walking. Avoid any heavy lifting or bending from the waist to get objects from the floor until you get approval to do so from your cataract surgeon. Swimming and water activities should be avoided for at least the first week after surgery because getting water in your eyes could increase the risk for infection. However, most patients are completely back to their exercise routines within one month after surgery.”

Eye Exam Test for Risk of Stroke

There may be a valuable new eye exam test for evaluating the risk of stroke. “I was pleased to learn about a possible eye exam text for identifying patients at high risk of stroke,” said Ivan Jacobs, M.D. of The Eye Care & Surgery Center. Researchers from the University of Zurich reporting in Ophthalmology, the journal of the American Academy of Ophthalmology outlined a results of a test called Ocular Pulse Amplitude (OPA) that can reliably detect carotid artery stenosis (CAS) or carotid artery narrowing, a condition that clogs or blocks the arteries that feed the front part of the brain which is a known risk factor for stroke. “I am hopeful that we will soon be able to perform this test as part of a routine eye exam,” commented Dr. Jacobs.

Each year, approximately 795,000 Americans suffer a new or recurrent stroke, and more than 137,000 of these people die as a result. People with severe CAS are much more likely to suffer stroke.”We would like to catch and treat CAS before that can happen, but because CAS has no symptoms and an efficient test is not currently available the disease often goes undetected,” explained Dr. Jacobs. 

"The results of the study show that ocular pulse amplitude (OPA) is a reliable, safe screening test for carotid artery stenosis," said Dr. Jacobs. “This is important as early detection of clinically significant CAS will go a long way to stroke prevention. At the moment there are some high-tech tests, such as magnetic resonance angiography and color duplex ultrasound, but they are expensive and not widely available-and certainly not part of a routine eye exam in our offices,” said Dr. Jacobs.

NJ Surgeon on Laser Cataract Surgery

“The laser allows us to further increase the control and precision developed over 30 years as a cataract surgeon in several stages of cataract surgery. We can design incisions that are more exact and reproducible, that are placed more precisely and seal more completely without stitches, lowering the risk of infection and increasing safety. 

We can also more accurately design and create the capsulorehexis, which is the circular opening that we make in the front of the cloudy lens to gain access to the cataract. This allows for safer and more efficient removal of the cloudy lens material, and allows for proper positioning of the lens implant. This is very important for achieving the best possible visual result. The ability to create a perfectly round and centered opening in the lens capsule is a major advance,” explained Dr. Confino. 

Dr. Confino further explained, “What is also quite meaningful with laser cataract surgery is our ability to reduce the amount of energy inside the eye. In order to remove the cloudy lens material of a cataract we use a procedure called phacoemulsification-which is a high energy ultrasound technique that breaks the material into fragments. With laser cataract surgery we use less ultrasound energy as we can chop up and soften the material with the laser and deliver a gentler cataract surgery procedure. This is particularly important for patients who have any other type of eye disease or problem including corneal problems, retinal problems and even diabetes and glaucoma.”

UV Eye Protection for NJ Spring & Summer

“As we get into the full swing of spring and summer outdoor activities we need to be aware of the need to use appropriate UV protection for our eyes,” noted Joel Confino, M.D. of The Eye Care & Surgery Center serving central and northern New Jersey. “May is designated as Ultraviolet (UV) Eye Protection Awareness Month and should be a reminder. The summer months are a great time to be outdoors-walking, hiking, swimming-going to the beach-and just enjoying our community. But, with more exposure to the sun and UV rays on a daily basis, we need to be reminded to use proper UV eye protection”, remarked Dr. Confino. Almost everyone is aware of the potential damage that sunlight and ultraviolet (UV) radiation can cause to their skin, however most people do not know or appreciate that ultraviolet light (UV) can also cause significant damage to eye health and vision.

Exposure to UV has been linked to many eye problems and in some instances is thought to quicken the onset of eye diseases such as cataract, age related macular degeneration (AMD), certain cancers on the delicate skin around the eye, Pterygia-a growth of white tissue on the surface of the eye-and even Photokeratitis, a type of sunburn of the cornea. “One of the most concerning aspects of UV eye damage and injury is that it is cumulative and often doesn’t become noticeable to patients for many years,” emphasized Dr. Confino. Some precautions regarding UV eye protection include:

• Always wear sunglasses that block 100% of UVA and UVB rays. Sunglasses without UV protection may shade the eyes but actually cause the pupils to dilate, allowing in even more harmful rays.
• Wear a brimmed hat along with sunglasses that block 100% of UVA and UVB rays.
• Choose wrap-around sunglasses to protect the eyes and the skin around the eyes.
• Be aware that although some contact lenses may offer UV protection, they cannot protect the entire eye and the skin around it.
• Choose sunglasses made of unbreakable polycarbonate, especially for children and for those who lead active lifestyles.
• Be sure to ask your doctor about the photosensitizing side effects of any medications that you might take.
• Always wear eye protection when using a tanning bed. According the American Academy of Ophthalmology, tanning beds can produce UV levels up to 100 times more than the sun, which can cause serious damage to the external and internal structures of the eye and eyelids.

Today we have wide range of UV protecting eyeglass lenses available to choose from. Eye protection from UV needs to be complete but doesn’t have to be overly expensive. High quality sunglasses should block out 99-100 percent of both UV-A and UV-B radiation. It is also important to have UV protection in everyday eyewear, which is readily available with a number of UV blocking eyeglass lens materials, specialized coatings and photochromic lenses.

NJ Patient Too Old for LASIK?

LASIK Surgeon Joel Confino, M.D. commented on whether a recent patient was just too old for LASIK. “I recently examined a 72 year old gentleman who wanted to know if he was too old for LASIK,” shared Dr. Confino a Corneal Specialist at The Eye Care & Surgery Center in Westfield, Warren & Iselin, New Jersey. “In reviewing his general health he was in good shape-and after his eye exam I was comfortable that his eye health, corneal health and tear film were quite good.
